Our office participates with Delta Dental, Cigna, Ameritas, Assurant, and Principal Insurance, Dentemax benefit plans.

As a courtesy to all of our patients, we will submit insurance claims to other benefit plans on your behalf, so you may receive reimbursement, but ALL procedures are expected to be paid in FULL prior to services rendered.

Please do not hesitate to call should you have any questions or concerns regarding your initial visit. Please bring your insurance information with you to the consultation so that we can prepare the necessary forms to be submitted to expedite your reimbursement.
